This is the week! Community Pediatricswill has become UW Health Kids Beaver Damon as of November 1. The namewill has changed and therewill be are some new faces but the providers and nurses you know will we are stillbe here, still providing the same great care, with some improvements!

IMPORTANT changes in how to contact us:
 

• The current previous PATIENT PORTALwill no is longer be active, please do not send any portal messages after noon on 10/31 as they will not be seen.There will be Patients and families can contact the clinic for information on how to sign up for UW MyChartif you have not already, whichis will be the new patient portal.
• The current phone number will still be active, and be forwarded for the time beingclinic phone number is 608-915-0150.
• The clinic will be closed on Friday 11/1 and will have fewer open slots the week of 11/4-11/8. Please be patient during this time of transition as all of our staff are being trained on the new computer systems and the information is being transferred. We will continue to do our very best to address the health needs of your children and will continue to have nurses and physicians available for phone advice at all times.
