Community Pediatrics has become UW Health Kids Beaver Dam as of November 1. The name has changed, and while there are some new faces, the providers and nurses you know are still here, providing the same great care—with some improvements!

IMPORTANT changes in how to contact us:
 

• The current previous PATIENT PORTAL is longer active, please do not send any portal messages after noon on 10/31 as they will not be seen. Patients and families can contact the clinic for information on how to sign up for UW MyChart. MyChart is the new patient portal.
• The current phone number is still active. The clinic phone number is 608-915-0150.
